Membership Information Forms

This is the Application form for all NEW and Reinstated Members.

NEW CHAPTER ROSTER CHANGE FORM
Please use this form to report any changes to your member roster including new members, reinstated members, suspended, demitted, and deceased. The complete form package contains two (2) pages.

YEARLY NON COMPLIANT MEMBER LIST
Membership Tax is payable through and by the local chapter and due no later than March 31 yearly. All members who desire to remain in the Order of Eastern Star MUST remit this tax by the date due or forfeit membership entirely. Members who forfeit membership and wish to reinstate must pay a $50 reinstatement fee as well as the $45 tax at the time of reinstatement.

DEMIT
This form should be handled by your Grand District Deputy. It is used for current members who would like to be transferred to a different chapter. When processing a Demit, you must obtain the signatures from both the original Chapter and the Gaining Chapter. Once these signatures are secured, send the Demit form to the Grand Office.

OES Certificate of Death
This form needs to be downloaded and completed when sending in a Death Claim on a deceased member.

OES Certificate of Burial
This form needs to be downloaded and completed when sending in a Death Claim on a deceased member.

This form needs to be downloaded and completed whenever  there is change in your membership roster. The completed form needs to be mailed or emailed to the Grand Office.

COVID-19 2023 Waiver

Upon the death of a member, certain benefits are given to the person(s) listed on the member’s OES Certificate of Beneficiary. Worthy Matrons should submit a Death Claim Packet to the Grand Office when requesting these benefits on behalf of the beneficiaries.

A complete Death Claim Packet will consist of ALL of the following:

  • Original copy of the member’s Death Certificate
  • Original copy of the member’s OES Certificate of Beneficiary
  • A completed OES Certificate of Death, and
  • A completed OES Certificate of Burial. (If the member did not belong to the Burial Department. this form does not have to be submitted in the packet.)

Matrons please remember, if your members are not current on their dues AT THEIR TIME OF DEATH, they will not be entitled to any benefits. You will not be able to pay up for a deceased member after their death so that their beneficiaries can receive the benefits.
